Senior Backend Engineer (GenAI / LLM Workflows)

Compensation: $170,000–$230,000 base + equity
Location: San Francisco, CA or (open to remote to start)

They’re now hiring a Senior Backend Engineer to scale the core services that power the platform and build new Generative AI / LLM-driven workflows clinicians will rely on every day.

The Role

This is a backend-heavy role for an engineer who enjoys solving real production problems: scalability, reliability, data integrity, and building practical AI-enabled workflows that directly support patient care.

What You’ll Do

  • Ship complex backend services using FastAPI with MongoDB and/or Postgres
  • Build APIs and infrastructure that interface with LLMs and Generative AI models
  • Leverage AI-assisted development tools (e.g., ChatGPT/Copilot-style tooling) to increase engineering velocity without sacrificing quality
  • Prototype MVPs quickly, then harden them into secure, scalable, reliable production systems
  • Improve backend architecture, observability, and code quality standards
  • Deploy and operate services using Docker and AWS
  • Collaborate with senior engineers and mentor junior developers as the team scales

  • 7+ years of professional software engineering experience with deep backend expertise
  • Startup experience shipping real features in high-growth environments
  • Experience designing distributed systems and/or data-heavy backend architectures
  • Experience working with Generative AI / LLM-powered systems in production or a strong desire to learn and build in this space
  • Strong product mindset and comfort partnering with cross-functional teams
  • Ability to operate in ambiguity and move quickly without sacrificing quality
  • High standards for reliability, clarity, and maintainability when real users depend on your code
