The Role

Join our dynamic team in Darwin! As the Senior Coordinator of Supports, you'll be at the forefront of implementing tailored NDIS plans for our clients. Reporting directly to the Senior Coordinator of Supports, you'll play a pivotal role in ensuring that all supports are seamlessly integrated into our clients' lives. Your day-to-day will involve maintaining regular communication with clients, prioritising their needs, and documenting progress. With a focus on efficiency and adherence to organisational policies, you'll manage your caseload with precision and care, making a tangible difference in the lives of those we serve.

What you’ll be doing

• Collaborate with clients and key stakeholders to develop support plans, focusing on their strengths.

• Foster positive relationships with clients and their families, while upholding professional boundaries.

• Oversee the implementation of NDIS Plans, ensuring service quality and budget adherence, and addressing any arising issues.

• Aid clients in comprehending their NDIS plans.

• Support clients in enhancing their ability to live independently.

• Promote client autonomy by facilitating their choice of service providers.

• Maintain high-quality documentation in client files, meeting quality standards

• Assist clients in overcoming barriers to accessing services within their existing funding.

• Keep the NDIS informed of any changes in circumstances and guide clients through the process of requesting a plan review and exploring new options.

What we are looking for

• Minimum Certificate Ill in Disability, Mental health, or relevant industry experience

• Proven experience working with people with autism, disability and/or mental illness

• Commitment to the human rights of people with disabilities and the principles of participation and inclusion

• Engage in evidence-based practice and prioritise the client's unique needs to achieve positive outcomes.

• Excel in communication and foster positive relationships with external stakeholders, including with family carers, service providers

• Exhibit excellent verbal and written communication skills.

• Display effective time-management skills to coordinate their weekly schedule and appointments.

To be considered for the role, you will also need to meet the mandatory compliance requirements which includes:

• NDIS Worker Screening Clearance

• Working with Children Clearance (Ochre Card)

• National Police Clearance (dated within 6 months or less)

• Current First Aid & CPR certificate

• NT Drivers License

• Evidence of Australian Work Rights (current visa or Australian Birth Certificate)
