You will be part of HPB’s Outreach team, which aims to create a Healthy Living Environment where healthy living becomes part of everyday life. The team works with healthcare clusters, community partners and local stakeholders to support residents, especially seniors, in staying active, independent and well.
We are seeking a dynamic Manager/Senior Manager to support the planning, implementation and scaling of pre-frail programmes. The role will focus on strengthening community-based preventive health efforts, connecting seniors to suitable lifestyle programmes, and supporting upstream interventions that delay or prevent frailty.
[What you will be working on]
As a core member of the team, you will help operationalise HPB’s approach to pre-frail programme management and scale-up. This includes coordinating existing programmes, identifying opportunities to expand reach, and working with healthcare clusters, AACs and community partners to support seniors in maintaining strength, balance, mobility and overall functional health.
You will develop skills in programme planning, stakeholder engagement, data storytelling, project management, cross-agency coordination and translating ground insights into practical actions that improve seniors’ health outcomes.
1. Programme Management & Operations
Oversee end-to-end delivery of frailty prevention programmes
Plan and coordinate programme schedules, resources, and deployment at scale
Ensure operational readiness and consistency across all programme runs
2. Vendor & Partner Management
Manage vendors, service providers, and delivery partners to ensure quality and compliance
Work closely with community partners (e.g. AACs, grassroots, healthcare clusters) to coordinate programme delivery and participant recruitment
Strengthen partnerships to expand reach, especially to inactive and at-risk seniors
3. Risk, Safety & Quality Assurance
Implement and enforce safety protocols for programmes involving higher-risk senior participants
Conduct audits and ensure adherence to operational and clinical safety guidelines
Manage incident reporting, escalation, and resolution
4. Customer Experience & Issue Resolution
Oversee participant feedback and customer care processes
Address operational challenges and service gaps in a timely manner
Ensure a positive and seamless experience for programme participants
5. Programme Scaling & Continuous Improvement

Successful candidates will be offered a 1-year contract.
