  • Salary: £34,434per annum (pro rata for part time)
  • Hours: 37 hours per week , flexible working options available
  • Contract: Fixed Term or Secondment to support the backlog of cases, ending once the work is complete (6 months).

Your role and responsibilities

  • Engage with customers to assess risk and understand their views on how interventions should be progressed.
  • Gather and analyse information from partner agencies, review records, and collaborate with professionals to support effective multi-agency decision-making.
  • Contribute to and attend strategy meetings, identifying additional interventions where appropriate.
  • Respond to calls on the Professional Consultation Line, providing advice and guidance to professionals on referral pathways.

You will need

  • Knowledge of adult social care, including safeguarding practice, and an understanding of the Care Act.
  • Ability to work in an organised, efficient and responsive way, managing competing priorities.
  • Confidence in using case management systems, ensuring accurate and timely recording.
  • Strong communication skills, with the ability to adapt your approach to a wide range of audiences.
