The Senior Director of Home Care Services is a key leadership role responsible for driving strategic and operational excellence across home care programs. Reporting to the leader of Post-Acute and Palliative Care Services, this individual will oversee multiple departments and ensure alignment with organizational goals related to clinical outcomes, patient and staff satisfaction, regulatory compliance, and financial performance.
This role will lead the development and execution of innovative programs, foster a culture of continuous improvement, and ensure adherence to industry best practices. The Senior Director will collaborate closely with internal stakeholders—including those within post-acute and palliative care services as well as hospital, health plan, and physician leadership—to support departmental success as well as systemwide goals and key performance indicators (KPIs).
In addition to operational leadership, this position also serves as the Nurse Senior for Post-Acute and Palliative Care Services, providing clinical oversight and strategic direction for nursing practice across the continuum of care.
Key Responsibilities:
Lead and manage home care operations, ensuring high-quality, patient-centered care.
Develop and implement strategies to improve clinical outcomes, operational efficiency, and patient satisfaction.
Ensure compliance with all regulatory and accreditation standards.
Foster a high performing, engaged workforce through effective leadership, development, and support.
Collaborate with cross-functional leaders to align home care services with broader system initiatives.
Identify opportunities for innovation and growth within all homecare services.
Education
Bachelor’s degree required.
License/Certification
Clinical background with RN licensure preferred.
Experience
5 years of healthcare experience.
5 years of leadership experience.
Significant leadership experience in Home Health, Hospice, or Post-Acute Services is required.
Proven track record of operational excellence, team leadership, and strategic execution.
Strong understanding of regulatory requirements and industry standards in home care.
Clinical background with RN licensure preferred.
