MINIMUM QUALIFICATIONS:

Senior Certified Nursing Assistant/Activities will ensure that the residents ADLs, daily assignments and an ongoing program of activities in accordance with the comprehensive assessment are completed during the shift while identifying and resolving needs. Reports directly to the Activities Director (Therapy Aide Supervisor).
Assist with developing and implementing an activities program, which improves or maintains physical, mental and social well-being with a varied work schedule that may include weekend, evenings, and holidays.
Assist with developing an activities plan for each resident in accordance with the inter-disciplinary team process.
Responsible for the documentation of that plan in the resident’s medical record.
Responsible for initial interview with new residents and input of resident data into the medical record.
Performing duties of the Activities Director when the Activities Director is unavailable.
Responsible for maintaining the activity space and requesting of, equipment and supplies for the program and ensuring resident safety for individual and/or group activities.
Observe all facility safety, infection control policies and procedures.
Complete required documentation. Consistently work cooperatively with resident, resident’s representatives, facility staff, physicians, consultants and ancillary service providers. Maintain confidentiality of resident.
Performs all other related duties in an effective, timely, professional manner as assigned. Attend all in-service programs classes as assigned and complete assignment. Participates in interdisciplinary team and other facility meetings as assigned. Follow Residents Rights’ policies as all times. Assume accountability for data contained in the employees’ handbook.
Perform other duties as assigned.
