GENERAL SUMMARY

The primary responsibilities are two-fold. One: oversight of case managers – ensuring all Standard Operating Procedures (SOPs) comply and all case management assessments and notes are submitted in a timely manner, including periodic case management coaching and development (i.e., training) with the Program Director. Two: provide case management services to residents in Justin’s Place Recovery Program. The Senior Case Manager will assist the resident in achieving self-sufficiency through advocacy, goal setting, assessments, and by connecting the resident to community resources and recovery support services. The Senior Case Manager will work one-to-one with residents to develop recovery plans with goals and objectives that will be monitored monthly. The Senior Case Manager will facilitate the senior modules (Modules 8 and 9) class weekly and assist residents as they go through Steps 8-9 and the Genesis Process. St. Matthew’s House is a faith-based 501(c)(3) charitable organization that exists to transform lives in a spiritual environment that is both compassionate and disciplined.

GENERAL

  • Coaches Case Managers in SMH core values
  • Provides coaching, mentoring, and accountability to residents preparing for leadership roles within the program (e.g., Program Assistants in Vocational Training Career Paths)
  • Works collaboratively with Program Director and Case Management team to identify resident readiness for Transition Track, Recovery Peer Leadership, internships, vocational advancement, and graduation.

SPECIFIC

  • Ensuring Case Managers comply with SOPs and Mission Tracker assessments
  • Overseeing routine case management coaching and development training
  • Provide case management services for Modules 8 and 9 residents, including accurate and timely documentation
  • Participates in team meetings and works with the team to provide spiritual meetings, 12 Step Recovery Meetings, church, and other activities that promote the values and principles of St. Matthew’s House
  • Assists in the coordination of resident transportation, including monitoring and distribution of the schedule
  • Prepare upper module residents for graduation, to include collaboration with the Alumni Director and the resident to create a Home Plan, and assist with any related housing transition
  • Manages monthly medical logs, including observation of medication distribution and accurately documenting in log and case notes as needed
  • Ensures residents are following/maintaining Justin’s Place guidelines as outlined in the resident Guidelines for Community Living
  • Knowledge of Mission Tracker and Case Management competencies, documentation, and Justin’s Place assessment procedures
  • Other duties as needed and assigned by the Program Director.

Requirements

Education: High School diploma required, Associate’s degree/Bachelor’s Degree/preferred with an emphasis on leadership or addiction specialties.

Experience: A minimum of 8 years’ experience working in the addiction/case management field. Florida State or National Peer Certification required

Confidentiality: Works with highly confidential information.

Mental/Visual Demands: Flow of work requires normal mental and visual capabilities.

Physical Demand: Must be able to stand for a minimum of 4 hours at a time and lift at least 25 pounds. Must be able to bend or kneel to the ground.

Working Conditions: Normal office environment.
