Job Description

Seeking responsible individual working collaboratively with individuals who have a Mental Illness to support their wellness, recovery and independence within our Supported Housing Program based in Brooklyn, Manhattan and the Five Towns. Responsibilities include: regular apartment visits, weekly phone calls, medication and medical monitoring, contacting client’s mental health and medical health teams on a monthly basis. In addition, this position entails documentation of rental payments, fire drill evacuations and apartment inspections. Position requires a Bachelors degree in the Human Services field with strong background working with individuals who have a Mental Illness, Masters preferred. This full time position is based in Brooklyn. Valid driver’s license and own car preferred.
