About the Role

IRT Greenwell Gardens is seeking to appoint an experienced and passionate Senior Care Coordinator to drive high-quality services. This role focuses on improving all areas of care while ensuring residents achieve an optimal quality of life, with a strong emphasis on meeting legislative and compliance requirements.

This position is offered on a full-time basis, working 38 hours per week, Monday to Friday.

Your work may include:

  • Supervise and manage the care team to ensure the standard of care provided allows residents the highest possible quality of life
  • Monitor all residents' care needs to meet their preferences in consideration of cultural, religious or ethnic background
  • Participate in case conferences and evaluate care plans as per policy
  • Complete resident assessments on admission and as needs/care changes
  • Ensure compliance with relevant legislation and guidelines are undertaken while assisting in maintaining budget and financial requirements
  • Ensure programs for meeting the needs of seniors are coordinated and developed according to IRT procedures and are designed to account for individual rights, needs and preferences
  • Constantly strive to attain improvement in all areas of work by participating in continuous improvement programs

To Be Successful You Will Have

  • Current registration as a Registered Nurse with AHPRA (Australian Health Practitioner Regulation Agency)
  • Minimum 5 years post-graduate nursing experience, with at least 2 years in aged care
  • Strong clinical background in wound care and palliative care (preferred but not essential)
  • Comprehensive knowledge of clinical issues related to ageing, including dementia-related disorders and the management of challenging behaviours
  • Working knowledge of the Australian National Aged Care Classification (AN-ACC) system
  • Previous experience in a supervisory or leadership role (desirable)
  • Proven ability to lead and support a multidisciplinary team
  • Excellent written and verbal communication skills
