Position Title: Senior Behavioral Health Integrated Clinician
Department: Behavioral Health
Supervisor: Manager of Behavioral Health
Status / Hours per week: Exempt / 1.0 FTE


Salary Range: The expected base salary for the position is $70,000-$112,000 per year

(commensurate with experience, licensure, etc.)

Primary Function:

The position serves in a senior clinical position within integrated services in the Behavioral Health (BH) Department and as the primary Teen Services clinician. The position is responsible for the clinical supervision of the BH integrated counselor position, delivery of integrated behavioral health services to the Primary Care, Urgent Care and OB/GYN Departments, primary integrated care for teen services and management of incoming referrals for BH triage.

Duties & Responsibilities:

  • Complete BH triage screenings for health center patients; provide needed crisis management, coordination for services and referrals for patients 6+ and above into adulthood.
  • Coordinate and consult with various treatment teams on the recommendations and treatment needed.
  • Provide counseling, either individual or family to a case load of patients.
  • Collaborate with the adolescent health team to identify, offer outreach and deliver care for those patients in need of psychosocial triage and services.
  • Attend school, community and civic association meetings as requested by UCHC and outside groups.
  • Develop and facilitate psycho-educational social support groups for adolescents and youth within the primary care site.
  • Develop and build upon existing collaborative relationships with external agencies related to adolescent social service needs.
  • Participate in primary care behavioral health consultation during work hours, be available for walk-in patients when scheduled for these open scheduled times.
  • Assist with coordinating patient care plans conducting ongoing collaboration with case managers including Community Health Advocates, Nurses and all other UCHC department staff and Providers.
  • Participate in Quality Assurance Performance Improvement projects, as needed for the integrated program area.
  • Participate in ongoing individual supervision.
  • Maintain and complete in a timely manner, all appropriate paperwork and documentation in compliance with the Behavioral Health Department policy and procedures for the program area, including use of the EHR and use of ICD 10 /DSM 5 coding as needed.
  • Provide support, clinical supervision, manage workload and new case assignments for a LCSW-level integrated counselor.
  • Assist in supporting HRSA AIMS staff with program development and building patient panel.
  • Manage incoming internal referrals to BH wait list by checking for language needs, insurance information, and appropriateness for triage or external referral or assignment.
  • Assist BH reception with support around appointment requests for new referrals, and incoming calls from patients looking to engage in BH services.
  • Assist BH team with management of the BH consultation line, serving as a leader in BH integration and consultation services.
  • Maintain flexibility for schedule to include one evening per week and one Saturday per month, as needed.
  • Perform additional duties in the department as needed.



Qualifications

Minimum Basic Knowledge:

  • Master's in Social Work with current LICSW licensure in the Commonwealth of Massachusetts, or LICSW within 6 months of employment start.
  • Solid working knowledge of computers.
  • Strong verbal communication skills.
  • Ability to work cooperatively with others both within the staff team and throughout other departments in an integrated care model.

Experience &

Qualifications:

• Clinician with solid assessment and crisis intervention skills.

• Knowledge of trauma-informed care and integrated care desired.

• Commitment to community service, along with desire to work with a high-needs patient population.

• Bilingual language skills in Cape Verdean Creole or Spanish desired.

Independent Action:

Work independently adhering to UCHC policies and procedures.

Supervisory Responsibility:

Supervision of BH integrated counselor and others as needed.

Define Access Level to PHI: Level 4: Authorized to access full health information. UCHC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
