The Senior Analyst position reports to the Manager, and will be responsible for supporting key initiatives and priorities across Connected Care . This role will leverage strong project and change management capabilities to support high priority and complex projects, aimed at transforming care experiences to improve patient outcomes.
Two early priorities include transforming UHN’s outpatient experience across 300+ clinics, impacting up to 1.5 million visits annually, and supporting the expansion of the Nurse Practitioner Led Connected Care Hub, a regional service delivery asset providing community based care, to include Active Remote Care Monitoring.
