BASIC PURPOSE OF THE JOB
Develop academic plans and provide educational instruction for patients receiving oncologic or hematologic care at Dolly Parton Children’s Hospital.

Minimum Education: Bachelor’s degree
Degree: Degree in education required
License/Certification Required: Valid Tennessee Educator License
Minimum Work Experience: 5 years of classroom experience

- Ability to teach across multiple grade levels and subjects.
- Experience in communicating with families and schools.
DUTIES AND RESPONSIBILITIES
- Collaborates with patient/family, care team, and school to create an academic plan for patients receiving oncologic care and/or chronic hematologic care during diagnosis, treatment and the transition to the school setting when possible.
- Provides school support services during hospital admissions and outpatient clinic visits that include educational instruction.
- Evaluates the academic plan during homebound instruction, transitions to schools, or at transition points (middle school, high school, college).
- Facilitates the plan for school re-entry with the patient/family, care team and school.
- Monitors academic performance through discussions with patient/family and school.
- Assists in the coordination of neurocognitive testing and incorporates neurocognitive test results into the academic plan.
- Facilitates the creation of 504 and/or Individualized Educational Plans (IEP) to promote academic success.
- Informs the family of education rights and access to accommodations.
- Completes school documentation alongside patient navigators.
- Participates in care conferences, interdisciplinary rounds, and psychosocial huddles.
- Establishes a supportive and effective relationship with the patient/family in the inpatient and outpatient setting and educates them on the school intervention specialist role during diagnosis and during the transition to follow-up care.

- Light lifting, pushing and pulling is required for 10-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
