The School and Community Outreach Associate is responsible for assisting School Linked Services & Community Outreach Program Manager with facilitating efficient, timely operations of screening referrals and maintaining relationships with the community and LAUSD and/other schools.
ESSENTIAL RESPONSIBILITIES:
- Processes and responds to school linked service requests from LAUSD and/other schools contracted with Wellnest.
- Maintains ongoing communication with referring party (PSW, School Psychologist) about the referral’s disposition and outcomes through exchange of monthly reports and as well through the county-wide Service Request Tracking System (SRTS) and Service Request Log (SRL) for DMH.
- Engages in community outreach events with SLS PM in school and community events, ie) back-to-school, resource fair, and health fairs.
- Assists in contacting referral source during outreach phase to engage families and students for services and gather documentation needed to process referral
- Maintains an accurate feedback status of school linked referral under internal systems and SRL on a daily basis
- Gather and upload required Wellnest, DMH, and Medi-Cal required documentation and IDs and ensuring these items are completed during screen and triage process by SLS PM
- Participates with other clerical and administrative support staff in routine and special tasks that support interdepartmental and general company functioning
REQUIRED QUALIFICATIONS:
High School diploma (or equivalent) required. At least 2 years of experience with entering and tracking of computerized data preferred.  Proficiency with software including Microsoft Word and Excel preferred.
CONDITIONS OF EMPLOYMENT:
Employee may be asked to pursue additional training when it is determined to be in the best interest of the Company. Employee must have the ability to use a personal vehicle with the required insurance coverage to complete the essential duties of their job and conduct agency business. Employee must maintain a valid California driver’s license and clean driving record that meets Wellnest’s insurance carrier’s criteria for liability coverage. In keeping with Company policy for all employees, the successful applicant must also complete a test for TB, and fingerprint clearance by the Department of Justice. Proof that you will be fully vaccinated against COVID-19 and boosted per eligibility guidelines by your employment start date.  Per the CDC and Department of Public Health, individuals are considered fully vaccinated for COVID-19: two weeks or more after they have received the second dose in a 2-dose series (Pfizer-BioNTech or Moderna or vaccine authorized by the World Health Organization), or two weeks or more after they have received a single-dose vaccine (Johnson and Johnson [J&J]/Janssen).  The Company will provide exemptions in accordance with applicable law.
WORKING CONDITIONS:
Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening and/or weekend hours for time-sensitive projects.

QualificationsCOMPENSATION AND BENEFITS
- Annualized
Salary Range: $39,520 to $47,840
- This position is also eligible for a prorated $2,500 bilingual Spanish differential
