Job Description

Schedule: Full Time – 80 hours/pay period – Monday to Friday – 8 a.m. to 4:30 p.m.

This position can work out of any of our Vivie Hospice locations in Alexandria, Fergus Falls, Wadena, Baxter, Morris, Waite Park, Coon Rapids, or West St Paul. This position is also available for our Ethos locations in Fargo, Bismark, or Grand forks

As a Hospice Scheduling Coordinator, you will:

  • Discern client services required as outlined in service agreements, per the plan of care, and for urgent requests.
  • Create and maintain visit schedules including supervisory visits per policy.
  • Work with the Hospice Manager of Clinical Operations and Clinical Care Coordinator to develop clinical territories and work to manage drive time/mileage.

Education and Experience Required

  • High school diploma or GED required; Associate degree preferred.
  • Experience with EMRs and knowledge of medical terminology preferred.
  • Strong organizational skills with the ability to manage multiple priorities independently.
  • Excellent attention to detail and data accuracy.
  • Strong communication and customer service skills.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with team members, clients, and visitors.

Key Responsibilities

  • Schedule Home Health Aide visits.
  • Schedule other discipline visits as directed by the Regional Hospice Manager of Clinical Operations
  • Schedule clinical visits per plan of care.
  • Ensure fiscal responsibility by following and adjusting clinical territories and taking drive time/mileage into consideration.
  • Schedule supervisions per policy.
  • Hold close communication with employees, clients and families.
