Summary:

Purchase District Health Department is seeking a community-centered and proactive individual to join the Rural Community Hubs for Chronic Care Innovation (RCH) program in the Purchase Area Development District.

Overview

This full time, term limited role is part of the Rural Health Transformation–RCH program, with Purchase District Health Department serving as the RCH Lead Organization for the Purchase Area Development District and the Foundation for a Healthy Kentucky serving as a statewide, neutral convening and implementation partner for the RCH program. The Rural Community Hub (RCH) Community Health Worker serves as a connector between Rural Community Hubs and the organizations, providers, and community partners operating throughout the Purchase Area Development District. Reporting to the Purchase District Health Department, the Community Health Worker supports partner recruitment, organizational onboarding, cross-sector coordination, and network development while helping partner organizations align their services with Hub priorities. This role also contributes frontline insight to Hub planning and implementation and participates directly in Hub-defined initiatives and collective efforts, helping reflect organizational capacity, gaps, and opportunities across the ADD.

This position plays a critical role in strengthening connections among providers, community organizations, and Hub partners while advancing the broader goal of improving health and well-being across rural Kentucky.

Community Health Workers help build and strengthen the network of organizations that serve residents by recruiting new partners, reducing barriers to collaboration, and enabling organizational voices to inform Hub planning and implementation. By supporting partner recruitment, coordination, network development, and organizational engagement, the RCH Community Health Worker helps create stronger connections across healthcare, social services, education, housing, and other community systems. The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP) — a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth. This project is 100% funded by the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS. The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics. Description:

Organizational Engagement & Hub Support:

• Build relationships with partner organizations and increase awareness of Hub services, programs, and opportunities
• Support partner recruitment, organizational outreach, and participation in local events
• Gather organizational input and elevate partner perspectives to Hub leadership
• Contribute frontline insight to Hub planning and implementation efforts
• Participate in Hub meetings, events, and Hub-defined initiatives and collective efforts
• Facilitate connections among partner organizations and initiatives to strengthen referral pathways, resource sharing, and coordinated support for communities

Partner Coordination & Navigation Services:

• Support coordination across healthcare providers, community organizations, and Hub partners
• Help partner organizations connect their services to health care, social services, and community resources
• Support organizations in navigating the Hub network, reducing barriers to collaboration, and following through on commitments
• Promote successful connections through warm handoffs and follow-up support
• Strengthen referral pathways across Hub partners and community organizations

Relationship Building & Collaboration:

• Build and maintain strong relationships with local organizations, service providers, and community leaders
• Coordinate with Hub partners to promote collaboration and a shared understanding of community needs
• Facilitate communication and knowledge sharing among Hub stakeholders
• Support operational excellence through partnership development and cross-sector collaboration
• Advocate on behalf of partner organizations and communities served to help build organizational capacity to sustain long-term health improvement efforts

Continuous Improvement & Documentation:

• Gather and communicate partner feedback to support service improvements
• Document outreach activities, partner engagement, follow-up actions, and emerging organizational trends
• Stay informed about local resources, community priorities, and Rural Community Hub practices
• Support reporting and continuous improvement efforts through accurate and timely documentation

Your Background:

• Community Health Worker certification or eligibility to obtain certification preferred
• Experience serving rural communities and/or Purchase Area Development District communities preferred
• Knowledge of chronic disease prevention, self-management support, or care coordination practices preferred
• Bachelor's degree or equivalent lived or professional experience in public health, social work, human services, community development, or a related field preferred
• 4+ years of experience in community health work, outreach, partnership development, care coordination, social services, or other community-facing support roles
• Deep commitment to improving health outcomes and quality of life for Kentuckians
• Strong interpersonal skills and ability to build relationships with community partners, organizations, and stakeholders
• Ability to manage multiple priorities while supporting both partner needs and broader Hub initiatives
• Familiarity with healthcare, social service, education, and other community resource systems
• Excellent written and verbal communication skills
• Ability to travel throughout the ADD for meetings, partner/community visits, and events

SALARY RANGE: $17-$19/hour based on qualifications and experience

The Purchase District Health Department (PDHD) is a public health agency with jurisdiction in five counties (Ballard, Carlisle, Fulton, Hickman, and McCracken) in the Purchase area. The mission of PDHD is to provide environmental, preventive, curative, and health maintenance services to area citizens.
