Through the Rock County Counseling Center, this position provides person-centered, trauma informed therapy and case management services to adults with complex and/or co-occurring behavioral health needs. Daily activities in this position include providing individual and group therapy services, completing intake assessments; completing safety assessments; documenting care provided; attending client staffings and administrative meetings; case management; discharge planning; collaboration with internal and external colleagues. Services are provided in-person in an office setting and in the community. Telehealth services are also provided based on client needs. This position supports and promotes the mission, vision and guiding principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to my Co-Workers."
