Job Category: NurseWe are currently seeking talented, compassionate RNs to provide GIP-level hospice care to patients at our Rebecca Fortney Inpatient Care Center, located on our campus in Pasadena, MD.  This position is PRN (as-needed), with a scheduling expectation of 2 12-hr shifts per month, weekend shifts included, and 1 'warm' holiday/1 'cold' holiday/year. (During the initial training period, this role will require 1 12-hour shift and 12 hours of flexible online learning per week until fully independent in the role).
This position pays $45/hr and is eligible for participation in our 401(k) retirement plan with a company match!
Main Responsibilities:
- Provide direct patient care sufficient to meet the patient’s total nursing need in accordance with the patient’s hospice plan of care assuring each patient receives treatments, medications, and diet as tolerated.
- Correctly identifies, communicates and collaborates on patient problems/priorities/issues based upon assessment data, establishes measurable patient goals and communicates initial/ongoing plan of care appropriately to interdisciplinary team. Recognizes changes in clinical status and reports them to the appropriate individual.
- Collaborates with patient/family when developing plan of care, adjusting communication style as necessary. Establishes priority for patient care based on immediate patient condition and mutually set patient needs/expectations.
- Ensures patient/caregiver teaching and patient/caregiver level of understanding and response to teaching are documented. Incorporates teaching into routine care, involving patient/caregiver. Incorporates learning needs in discharge plan.
- Completes all electronic documentation, at the point of care or in the patient care corridor, following the appropriate protocol.
- Assists Field Teams when needed

QualificationsYour

Qualifications
- Current Maryland RN license or accepted reciprocal out-of-state nursing licensure; and/or CHPN certification preferred
- 2 or more years of nursing experience in med/surg or home care required, with experience in hospice, end-of-life, or critical care experience preferred.
- Hospice certification (CHP) preferred.
- Strong and creative, problem-solving skills with the ability to prioritize and delegate.
- Familiarity and comfort with documenting in an Electronic Medical Record.
- Valid driver’s license with an acceptable driving record and reliable transportation
- Ability to meet the physical demands of the position with or without reasonable accommodation.

Through Chesapeake Life Center, we operate two weekend camps for grieving children and teens. We are unique in that we provide grief support services to anyone in the community seeking support including those who have experienced a traumatic or sudden loss. The scope of services far exceeds that which is provided for in the Medicare Hospice Benefit.
