đź’ˇ About the Role

In this role, you will:

  • Undertake initial holistic nursing assessments in partnership with the client, their family and the care team.

  • Develop individualised care plans in partnership with the client, their family and other healthcare professionals Effectively communicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff

  • Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.

  • Support staff by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.

📚 Experience and Qualifications We’re Looking For

  • A learning disabilities nurse with an active NMC pin

  • Experience leading community teams to support individuals with learning disabilities, autism and mental health needs

  • A valid drivers license and access to a vehicle
