RN Unit Manager
The RN Unit Manager is responsible for coordinating and overseeing resident care throughout the continuum of services in a long term care setting. This role ensures individualized care planning, effective transitions of care, regulatory compliance, and collaboration with the interdisciplinary team to promote optimal clinical outcomes and quality of life for residents.
Key Responsibilities
• Coordinate resident care from admission through discharge, including care transitions and discharge planning
• Conduct comprehensive nursing assessments and ongoing evaluations of resident needs
• Ensure development, implementation, and updating of individualized care plans
• Collaborate with physicians, nursing staff, social services, therapy, dietary, and other interdisciplinary team members
• Monitor resident progress and outcomes; identify changes in condition and coordinate appropriate interventions
• Serve as a liaison for residents and families, providing education, updates, and support
• Assist with insurance coordination, authorizations, and communication related to resident care (as applicable)
• Ensure documentation supports medical necessity, care planning, and regulatory requirements
• Participate in care conferences, quality assurance, and performance improvement initiatives
• Support survey readiness and compliance with state, federal, and CMS regulations
• Promote resident rights, dignity, and person centered care
Required Qualifications
• Current, active Registered Nurse (RN) license in good standing in Ohio
• Minimum of 2 years of nursing experience, preferably in long term care, skilled nursing, or care management
• Strong clinical assessment and care coordination skills
• Knowledge of CMS regulations and long term care compliance standards
• Excellent communication, organizational, and problem solving abilities
Preferred Qualifications
• Previous experience as a Unit Manager, Admissions Nurse, or Care Coordinator
• Familiarity with MDS processes, discharge planning, and transitional care
• Experience with electronic medical record (EMR) systems
• Participation in survey preparation and quality improvement initiatives
• BSN preferred
Physical & Work Requirements
• Ability to perform essential nursing duties
• Ability to move throughout the facility for assessments and meetings
• Ability to sit and work at a computer for documentation and coordination activities
