Responsible for the coordination of the care and management of the referrals into advanced organ disease programs and pre-transplant candidate. This includes the initiation and completion of the evaluation, coordination, and timing of testing to determine candidacy for transplant or advanced therapies. Must be able to function with considerable independence in the management of the referrals and pre-transplant patient.
Responsible for promoting the health and safety by ensuring the ongoing communication and collaboration providers and other multidisciplinary team members.
Maintains accurate and current data base on each patient recording all laboratory, diagnostic tests, biopsy results and changes in medications.
