This position is not remote and will be located at the UH Management Services Center in Shaker Heights, OH. It is full-time, 36 hours per week, on straight night shift rotating weekends/holidays.
Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management.
Essential Duties:
1.Coordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resources
2.Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes
3.Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations
4.Displays leadership that influences optimal clinical and operational outcomes
5.Delivering exceptional customer service while facilitating communication across multiple disciplines
6.Possess and obtain working knowledge of external and inter-facility protocols and standards of practice
7.Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach
8.Responsible for promoting a Best Place to Work Culture and positive and professional work environment
9.Radio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care Transport
10.Dispatch of UH Critical Care Transport ambulances
11.Other duties as assigned
Experience:
1.1 year of RN experience required
2.ICU or ED experience preferred
