Our Community Care Record platform enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs. Path Assist is our tech-enabled community health worker program for HRSN utilizing an evidence-based, structured intervention. Our goal is simple: increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spending.
The RN Supervisor leads a distributed team of RN Assessors who travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. This is a player-coach leadership role: you own your team's quality, productivity, compliance, and professional development, and you serve as the senior clinical resource for the assessment work. A central part of the role is protecting the health plan's completeness and quality standards by auditing your team's work before it is submitted. Like the assessors you lead, the team's scope is assessment and documentation with a clean handoff to the health plan's care management team, not care plan development or ongoing care management.
Responsibilities:
Team Leadership & Development
- Help with hiring, onboard, train, coach, and retain RN Assessors
- Provide day-to-day supervision, regular one-on-ones, and performance management, including field observation and ride-alongs.
- Build and maintain a coverage plan so assessment timeliness is protected during PTO, leave, and turnover.

- Reinforce field-safety practices for independent in-home visits and support nurses working alone in the community.
Clinical Quality and Audit
- Conduct rigorous pre-submission audits of assessments (Uniform Core, Functional, and Comprehensive), coding, and SOAP notes to intercept errors before final submission to the health plan's review team.
- Ensure documentation is completed in GuidingCare within the 48-hour turnaround and that the team meets the health plan's completeness and quality standards, as measured through review and audit.
- Identify recurring error trends and drive corrective action and targeted education across the team.
- Support timely responses to review and audit findings within contractually required timelines.
- Serve as the subject-matter expert on the assessment process, the tools, and applicable regulatory, privacy (HIPAA), NCQA, and care management standards.
Operations and Productivity
- Manage caseload distribution, visit scheduling across the daily slot structure and shift patterns, and territory and route planning to balance workload and completion targets.
- Track and report on team KPIs, including visits completed, assessment timeliness, accuracy, and submission deadlines.
- Partner with the scheduling team to keep assessor schedules full and efficient, and monitor productivity against targets.
Stakeholder Partnership and Compliance
- Partner with the health plan's program leadership through operations touchpoints and office hours, and collaborate with internal teams.
- Support compliance requirements, including workforce exclusion checks and timely completion of compliance, fraud-waste-and-abuse, and HIPAA training.
- Escalate clinical, compliance, or client concerns appropriately and follow through to resolution.

Requirements
Qualifications & Skills:
- Active Registered Nurse (RN) or higher licensure (e.g., Nurse Practitioner) in good standing with the Commonwealth of Massachusetts.
- Bachelor of Science in Nursing (BSN) preferred; Associate of Science in nursing required.
- Minimum of 3 years of RN experience, including home health or community-based assessment work.
- Prior supervisory, team-lead, or formal mentorship experience.
- Strong quality-assurance and audit skills, with the ability to identify trends and coach to them.
- Excellent coaching, communication, organizational, and reporting skills.
- Comfort documenting and working in an electronic platform via a secure virtual desktop (GuidingCare), and leading a remote, field-based team.
- Valid driver's license, personal vehicle, and verifiable insurance are required for field travel.
- Experience conducting or auditing assessments (for example UCA, FA, Comprehensive, MDS, or OASIS); assessment-related certification such as RAC-CT is preferred.
- Prior experience with dual-eligible populations, Medicaid, or Medicare managed care is preferred.
Working Conditions:
- Must reside within a commutable distance of the assigned Massachusetts territory.
- Field-based and hybrid; regular travel for field oversight, ride-alongs, and team support.
- Exposure to weather and varied home environments during field visits.
- Valid driver's license, vehicle, and verifiable insurance are required; employment is conditional on a successful driver's license record check and verified insurance, maintained throughout employment.
- Influenza vaccination is required during flu season (October 1 through March 31) as a condition of employment, consistent with the health plan's requirement for staff serving members in the home. Employees hired during flu season must complete the required vaccination and provide proof of immunization within 30 days of hire.
