Job Description

Provides on call support between 5 pm and 8:30 a.m. (Monday through Friday). Weekend coverage (from Friday at 5:00 p.m. through Monday at 8:30 a.m.). and holiday coverage. Weekend day coverage from 8:30 a.m. to 5 :00 p.m. The role takes all calls from the answering service and responds to them, documenting as appropriate in the patient record, the call, interventions and the outcome of the call. Ensures that admissions and visits scheduled for the times on call are conducted and when not ensures that there is documentation of explanation.

Responsibilities:

  • Is accessible by telephone during on call hours in the event that a home visit needs to be made.
  • Assesses actual and potential problems, needs and resources of the patient and family through observations, interviews, and analysis of records.
  • Receives all calls via the answering service, with follow-up to the caller, responding to questions , contacting the physician as necessary. Collaborates with the patient/family in revising or altering the plan of care.
  • Assigns a nurse or therapist to provides direct skilled nursing care according to prescribed treatments, e.g. change foley catheters, cardiopulmonary assessment, wound care and restorative nursing.
  • Communicates with the health care team or appropriate manager at the start of the next business day regarding the call, any interventions and the outcome of the intervention.
  • Documents services provided according to Agency protocols.
  • Communicates with other health and social agency personnel to assist in planning, implementing, and evaluating services to the patient and family.
  • Utilizes opportunities to involve patient/family in individualized teaching plans which promote and maintain wellness.
  • Ensures visits scheduled are completed, and when not completed ensures that there is a documented explanation.
  • If a Start of Care is not completed, may pull a case scheduled for the next day back to admit.
  • Supervises Home Health Aide/Home Attendants with regard to their duties, both in the patient's home and via telephone.
  • Participates in continuing education programs and provides feedback to assist management in program planning and development.
  • Acts as a patient advocate.

Qualifications:

  • Current NYS Registered Nurse License
  • Home care experience (required)
  • Must be familiar with Home Care Home Base
  • BSN

Pay Range


$500.00 per day ($62.50 per hour)
