As an Admissions RN, you'll focus on Start of Care visits, using your assessment skills, clinical judgment, and OASIS expertise to ensure every patient begins their home health journey with confidence. Experienced Clinical Managers who understand the complexity of Start of Care nursing. A collaborative interdisciplinary team that's always available when you need support. What You'll Do Complete comprehensive Start of Care (SOC) assessments in patients' homes. Accurately complete OASIS documentation and establish individualized plans of care. Educate patients and caregivers about diagnoses, medications, and home health services. Coordinate care with physicians, therapists, case managers, and other members of the care team. Ensure smooth transitions from hospital, rehabilitation, or skilled nursing facilities to home. Maintain timely and accurate documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical assessment and critical thinking skills. Home health and OASIS experience preferred. Excellent communication and organizational abilities. Ability to work independently in a field-based role. Reliable transportation.
