Help patients move from injury to recovery in our Orthopedic Clinic, supporting sports medicine and orthopedic trauma care!

What you'll do

In this role, you’ll coordinate follow-up care after emergency room visits or emergency surgery, assist with surgery scheduling, and provide patient education before and after surgical procedures. You’ll also coordinate follow-up appointments, physical therapy referrals, and other postoperative needs and resources to help patients navigate each step of their recovery.

Through in-person visits, online communication, and phone support, you’ll serve as a key connection between patients and their care team, helping patients feel informed, supported, and prepared throughout their care.

This is an in-person, on-site position.

Qualifications:

  • Current Montana RN License or a multi-state compact license with authorization to practice nursing within the state of Montana; required.
  • Minimum of one (1) of the following required:
  • Bachelor’s degree in a related field and four (4) years’ related experience.
  • Minimum of six (6) years’ experience in a related area.
  • Minimum of four (4) years’ recent specialty experience in a minimum of one (1) of the identified specialty areas required:
  • Clinical - such as inpatient post surgical floor experience
  • Orthopedics
  • Surgery, OR or pre/post operative experience
  • RQI Provider within 7 days of hire/transfer or before independent patient care, whichever comes first.
  • Knowledge of professional practice standards and regulatory requirements as related to specialty care area required.
  • Knowledge and experience with electronic health records required.
  • Must function with a high degree of autonomy, strong communication and interpersonal skills. Possess the ability to develop and implement community education, outreach programs/workshops and seminars as defined for assigned specialty area required.
  • Possess ability to comprehensively assess patient’s psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s) required.
  • Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams required.
  • Promotes excellence through setting high standards and providing high quality outcomes.
  • Ability to act with integrity, kindness, and exhibit empathy.
  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

Job Specific Duties:

  • Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.
  • Works closely with members of the multidisciplinary care team and their recommendations. Exhibits professionalism and accuracy in interfacing with providers, clinic staff, patients, and family members involved in the plan of care.
  • Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable. Assesses the physical, psychological, social and spiritual needs of the patient and family.
  • Identifies and assesses the learning needs of the population under care and/treatment. Builds therapeutic and trusting professional relationships with patients, families, and caregivers. Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).
  • Assists with the organization of the specialty interdisciplinary team meetings and facilitates the team’s recommendations in a professional and timely manner.
  • Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).
  • Works collaboratively with all stake holders to identify patient needs and assists in care and services, both medical and social, throughout the continuum.
  • Collaborates with the navigation team and leadership to optimize processes and metrics to; reduce avoidable ED visits, inpatient admissions, hospital readmissions, cost of care, gaps in care, increase patient satisfaction, clinical outcomes, and provider satisfaction as defined for assigned specialty care area(s)
  • Assesses education needs and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.
  • Facilitates the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.
  • Actively participates in activities that add value to organizational objectives and workflows, such as process improvement projects and/or community-related initiatives to include community health fairs, workshops, etc.

Maintains regular and consistent attendance as scheduled by department leadership.

Shift:

Day Shift - 8 Hours (United States of America)

Schedule: Full-time, 40 hours per week, Monday - Friday, 8-hour day shifts.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
