
St. Mary’s Medical Center seeks a full-time RN to work in the CVT Outpatient department. This individual will work collaboratively as part of a care team to address patients’ needs and create a patient -centered plan of care. Under the direction of the provider, the nurse will monitor the health and well-being of patients using certified health technology. Through various forms of communication, develop a comprehensive care plan designed to promote preventive care, address barriers to care, and support self-management of chronic diseases based on clinical standards of care. Collaborates and facilitates care based on identified needs of the patient.
Graduate of an approved School of Nursing. Current licensure as a Registered Nurse (RN) in the state of West Virginia or a valid compact state nurse license. Experience preferred but not required. Knowledge of current nursing procedures. BLS required. A minimum of two (2) years of experience in a clinical and/or case management nursing preferred. Position requires strong verbal and written communication skills. Must be compassionate, able to build strong relationships with patients and other caregivers, work collaboratively with others, solve problems and be flexible and technology proficient.
