Position Purpose:
In accordance withthe Mission, Core Values andVision;the Nurse Liaison Home Infusion will be a Registered Nurse responsible for assessing patients and their suitability for the Home Infusion program. This position will assume responsibility for verifying thatappropriate pre-certificationdocumentation is available to ensure reimbursement, quoting rates for therapy within established guidelines, and coordinating care with applicable agencies and the Mount Carmel Infusion Services pharmacy staff. This person will function witha high levelof independent judgment, using skills and knowledgeacquiredthrough education and customer service expectations to enhance patient care. In addition, the nurse will contribute clinicalexpertiseto organizational planning and decision-making and may also be involved in business development activities as they relate to business expansion or new program development.
What You Will Do:
Maintains daily communication with case managers/discharge planners and units to help ensure that patients referred for services are followedup withina timelyand efficient manner.
Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of Mount Carmel Infusion Services.
Provides andutilizesclinicalexpertiseand skills toassistreferral sources &/or patients with:
The identification ofappropriate candidatesfor home infusion &/or home health care
The identification of the patients’ and/orcaregiversability to provide care
Patienteducation and trainingregardingtherapy and supplies, as wellas,clarifying expectations of care in the home.
Medication connection at the hospital / facilityin order toexpeditethe discharge into the home setting, as appropriate
Coordinates and communicates all discharge planning activities for new patient referrals with Mount Carmel Infusion Services personnel and referral sources.
Appropriately documents the coordination of care activity including, but not limited to:
Homeinfusion &/or nursing services utilized
All communication with patients, referral sources,physiciansand Mount Carmel Infusion Services personnel
All patient &/or caregiver education & training
Interactswith healthcare team members to answer any questions related to home infusion and community-based services as they plan for patient discharges from the acute care setting.
Functions as a liaison between nursing and other disciplines and departments in resolving patient issues.
Minimum Qualifications:
Education: Bachelor’s degree in nursing (BSN) or Registered Nurse (RN) from an accredited college or school of nursing with equivalent experiencerequired
License / Certification: Current license to practice as a Registered Nurse in the State of Ohiorequired
Current Certified Registered Nurse Infusion preferred
(CRNI) credential preferred or be willing to obtain within 6 months of hire
Must have valid and current Ohio Driver’s license and reliable transportation to and from work site
BLS Healthcare provider trainingrequired
Experience: Must have a minimum of two (2) years' experience in infusion care setting and experience with processing referrals for homecare and pre-certification
