Registered Nurse (RN) | Family Medicine - La Crescent/Minnesota Hub

Primary Location: La Crescent, MN

Secondary Locations: Spring Grove, Harmony, Houston (MN) - as needed

FTE: Part-time, 0.8 (64 hours per two weeks)

Hours: 8-hour shifts between the hours of 7:00 a.m. and 4:30 p.m.

Schedule: One set day off per week (Thursday) | No weekends, holidays, or on-call responsibility

What You'll Do

• Conduct nurse-only visits, including Medical Annual Wellness Visits (MAWs)
• Triage patient concerns through phone calls, MyChart messages, and walk-in assessments
• Deliver direct patient care, including wound care, injections, anticoagulation management, and ankle-brachial index (ABI) testing
• Assist with procedures and specialized patient care needs
• Provide patient and family education while coordinating care across the healthcare team to ensure positive patient outcomes
• Help maintain a positive, patient-centered environment across the Minnesota Hub clinics

What You'll Bring

Required Qualifications

• Minimum of an associate degree in nursing
• Current Registered Nurse (RN) licensure in the state of Minnesota
• Ability to obtain and maintain RN licensure in Wisconsin and Iowa within 3 months of hire
• Current Infant-Adult Basic Life Support (BLS)for Health Care Providers certification with hands-on training through the American Heart Association or American Red Cross.

Preferred Qualifications

• Previous experience in the primary care setting: Family Medicine, Internal Medicine, or Pediatrics
• Experience with patient triage
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to effectively manage multiple priorities in a fast-paced environment
