
What You Will Do
The Valve and Structural Heart RN Navigator will coordinate and manage patient referrals, including contacting patients, completing intake documentation, scheduling appointments, and ordering procedures, laboratory work, and other diagnostic testing. This role is also responsible for following up with patients to ensure all required testing is completed in a timely manner.
In addition, the RN Navigator will manage Structural Heart meeting documentation to ensure patients are reviewed by the multidisciplinary team. The position includes communicating team recommendations to patients, coordinating procedures, and supporting the ongoing plan of care.
Hours This is a hybrid role that requires ~2 days per week onsite at CMC and the remainder of the week you will follow the physician to clinic or work from home (depending on physician/dept schedule). Flexible day time hours 5 days a week (Mon-Fri) and the ability to stay late on occasion of clinic runs late.
