RN-Patient NavigatorEnhances the patient experience by providing a seamless navigation process from diagnosis through end of care. Responsible for collaborating with physicians and members of the multidisciplinary teams to triage, coordinate, and consistently manage patient care by assuming the role of resource person for the patient and the multidisciplinary team. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.
Education and Experience Required:
Two (2) years of work experience as a Registered Nurse.
Certifications, Licenses, or Registration required:
Valid RN License
Preferred Qualifications:
Specialty or case management experience
Knowledge, Skills, and Abilities:
Knowledge of clinical nursing care procedures and protocols. Verbal and written communication skills. Interpersonal skills. Knowledge and understanding of computer systems, tools and programs. Ability to work in a team environment.
