The Registered Nurse will promote wellness, improve outcomes, and support self-care within a defined patient population, including those in Assisted Living Facilities and Private Homes. The RN will provide patients with chronic conditions with support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in their homes, assisted living, and skilled nursing facilities. Coordination of prescription refill requests and patient education coordination in collaboration with the physician staff. Commitment to patient outcomes leads to high performance levels despite workload changes.
Minimum Qualifications:
Bachelor’s degree in Nursing, Associate’s degree in Nursing, Diploma RN, or Master’s degree in Nursing.
Must be a graduate of an accredited RN nursing program
Valid Registered Nursing License
Minimum of 1 year of Registered Nursing experience;
BLS certification within 30 days of hired.
Basic computer skills.
Excellent communication skills.
Strong work ethics and accountability.
Preferred Qualifications:
Charge RN or Leadership experience.
One (1) year of clinic or ambulatory setting experience.

Responsibilities:
Triage, assessment, planning, implementation, and evaluation of nursing care and patient education either by telephone or in the patient's home.
Evaluates the progress of patients toward the attainment of desired outcomes.
Administers medication following clinic standards and the Nurse Practice Act.
Verifies and evaluates care delivered to ensure care is consistent with standards and regulatory requirements.
Coordinates care and/or follow-up for patients who receive care in assisted living facilities.
Coordinates patients’ prescription refill requests and patient education coordination in collaboration with the physician.

Provides effective patient/family education/discharge planning;

Delivers patient care using the principles of the defined nursing modality;
Delegates care based on UAMS standards, legal and regulatory requirements, specific job descriptions, and individual levels of education and training;
Contributes to a positive work environment conducive to clinical education while serving as a resource for student nurses and staff.
