The Nurse Navigator is a leader providing support, mentorship, and guidance to those who provide direct patient care. The Nurse Navigator coordinates the delivery of patient care by assessing care requirements of the individual patient and makes recommendations based on severity of illness, intensity of service and skill level. The Nurse Navigator ensures patients receive timely, safe, effective and high quality care. They coordinate the patient's plan of care with the bedside nurse. The Nurse Navigator sets the expectations regarding professional conduct and serves as a resource for the patient care unit. The Nurse Navigator will hold direct and indirect care providers accountable for their professional responsibilities, and adherence to regulatory guidelines. The Nurse Navigator is responsible for management for the assigned inpatient, and outpatient population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety, and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Responsibilities include; pre-op education, daily rounding while the patient is in the hospital, patient discharge disposition, and follow-up care. Registered Nurse (RN) Nurse Navigator - Mercy Clinic Orthopedics Southfork

Location: 12700 Southfork Road, Suite 100, St. Louis, MO 63128
Schedule: Full-Time, Monday-Friday (40 hours/week)

Mercy Clinic Orthopedics Southfork is seeking a compassionate and experienced Registered Nurse (RN) Nurse Navigator to serve as a trusted guide for patients throughout their orthopedic care journey. In this role, you will coordinate care, provide education, and support patients from initial consultation through surgery, recovery, and follow-up care.

Working alongside orthopedic surgeons, advanced practice providers, therapists, and interdisciplinary teams, you'll help ensure timely, high-quality care while delivering an exceptional patient experience.

What You'll Do

  • Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
  • Educate patients and families regarding diagnoses, procedures, treatment plans, and post-operative care.
  • Facilitate communication between patients, providers, and care teams to ensure seamless care coordination.
  • Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
  • Support pre-operative planning, discharge coordination, and post-operative follow-up.
  • Monitor patient progress and address barriers to care.
  • Maintain accurate and timely documentation in EPIC and other clinical systems.
  • Track outcomes and quality metrics to support evidence-based practice and improved patient outcomes.
  • Participate in quality improvement initiatives and program development efforts.
  • Serve as a clinical resource for patients, families, providers, and staff.

Qualifications

Required

  • Graduate of an accredited school of nursing
  • Current Registered Nurse (RN) license in the state of practice
  • 2-3 years of nursing experience
  • Basic Life Support (BLS) certification

Preferred

  • Bachelor of Science in Nursing (BSN)
  • Orthopedic, surgical services, care coordination, or nurse navigator experience
  • Advanced Cardiac Life Support (ACLS) certification

Physical Requirements

  • Ability to push, pull, and/or lift up to 50 pounds
  • Prolonged standing and walking throughout the workday
  • Ability to bend, reach, kneel, twist, squat, and perform other physical movements as necessary
