Job Objective:

Assures that communication with each caller will be handled in a professional and tactful manner. When appropriate, the caller’s symptoms will be assessed and triaged using the nursing process and approved guidelines in order to assist in obtaining the appropriate level of care and/or self-care advice. This position will also assist and be a resource for all aspects of the medication refill, prior authorization process and patient navigation through the health system. This role will communicate and work effectively with patients and various members of the healthcare team including clinical support staff and providers. Education:
Required: Bachelor of Science in Nursing (BSN) or Master’s in Nursing
Licensure/Certification:
Required: California RN License; American Heart Association BLS, AAACN Telephone Triage Course within 90days of hire
Experience:
Required: Five (5) years of experience as an RN in a clinical setting
Preferred: Experience in Emergency Department and with electronic health records Reports To: Clinical Manager, Clinical Director Supervises: N/A Ages of Patients: Neonate/Infant, Pediatric, Adolescent, Adult, Geriatric Blood Borne Pathogens: Minimal/ No Potential

Basic computer skills and knowledge, Knowledge and ability to function in a professional role and serve as a professional role model, Knowledge of current nursing, medical and health trends and practices in rendering of telephonic patient care, Must be capable of making independent nursing decisions, Strong organizational, interpersonal, written, verbal, teamwork, critical thinking skills

Essential Responsibilities

1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
2. Performs a nursing assessment and patient triage telephonically which can include prioritizing cases, providing medical advice, making complex referrals, problem-solving complex cases and collaborating with healthcare providers.
3. Thoroughly completes documentation utilizing the appropriate computer software in compliance with the approved policies, protocols, procedures and/or workflow.
4. Maintains positive interpersonal relations with peers, patients, families and providers.
5. Interacts with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
6. Considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Provides patients linkages to services to promote a pathway to care and positive outcomes.
7. Attains knowledge and competency that reflects current evidence based practice for nursing care. Facilitates health and disease patient education. Supports patient self-management of disease and behavior modification interventions.
8. Systematically enhances the quality and effectiveness of nursing practice using the Relationship Based Care model and shared governance for decision making.
9. Practices within the scopes and standards set forth by professional organizations specific to specialty.
10. Supports additional projects related to care coordination across the health system.
11. Performs other duties as assigned.
