In Conjunction With The Medical Plan Of Care, Independently Performs Systematic And Continuous Assessment Of The Physical, Emotional, Clinical And Psychosocial Needs Of Assigned Patients And Families; Plans And Implements Appropriate Interventions; And Evaluates Effectiveness And Adjusts Plan As Indicated In A Timely Manner In The Provision Of Safe And Effective Care.
Responsibilities:

•Performs Coordination Of Care In Collaboration With Healthcare Team
•Evaluates Patient Response To Plan Of Care And Updates And Revises Plan Accordingly
