In Conjunction With The Medical Plan Of Care, Independently Performs Systematic And Continuous Assessment Of The Physical, Emotional, Clinical And Psychosocial Needs Of Assigned Patients And Families; Plans And Implements Appropriate Interventions; And Evaluates Effectiveness And Adjusts Plan As Indicated In A Timely Manner In The Provision Of Safe And Effective Care.
Responsibilities:

•Formulates Patient Plan Of Care Based On Assessment Of Patient Needs
•Implements Plan Of Nursing Care In A Safe And Effective Manner
•Performs Coordination Of Care In Collaboration With Healthcare Team
•Evaluates Patient Response To Plan Of Care And Updates And Revises Plan Accordingly
•Accurately Documents Patient Treatment And Response And Maintains An Accurate Account In Patient Record•Demonstrates Professional Behavior
