
A RN Medical Care Manager contributes to AHF by blending the Mission and Core Values with care coordination processes and functions. The PIC RN demonstrates strong clinical skills, proactive thinking, comprehensive knowledge of healthcare rules, medications and supportive services, with a special focus dedicated to HIV/AIDS members, ensuring that all members have access to a comprehensive set of services based on their needs and preferences across the continuum of care.
A RN Medical Care Manager enhances the reputation of AHF by conducting comprehensive health assessments up to 180 days, establishing a coordinated personalized care plan with the members and proivders, reviewing and education of members on medication adherence, addressing behavioral health and social needs, emphasizing the importance of medical appointment compliance, addressing NYS HIV Quality Measures, counseling and education to achieve HIV viral load suppression, support members through transitions of care and participation in monthly care management meetings.

The RN Medical Care Manager will work as a part of a close knit team to promote healthy behaviors and individual member care plan adherence through onsite and offsite outreach/engagement, discussions of STI impact on care, and adherence to primary care and specialist treatment plan(s). PIC RN will cultivate and facilitate care coordination for healthplan members including developing a personalized care plan, closing care gaps and engaging in opportunities to increase knowledge of MetroPlusHealth’s PIC plan benefits, and community resources to facilitate access to care. The PIC RN collaborates with the members’ primary care provider and leads the interdisciplinary team planning to ensure that health plan members receive medically necessary and appropriate, individualized, and enhanced healthcare.
Licenses and Certifications: NYS RN License required. HIV experience preferred. Care Management experience preferred. Bilingual Spanish preferred
