The MDS Nurse Coordinator (RN) is responsible for the timely and accurate completion of Minimum Data Set (MDS) assessments in accordance with:

  • RAI Guidelines,
  • CMS regulations,
  • OBRA requirements,
  • Medicare and payer guidelines,
  • and state/federal compliance standards.

This role collaborates closely with Nursing, Therapy, Social Work, Dietary, Activities, Business Office, and facility leadership to ensure accurate assessment coding reflective of resident acuity and services provided.

Responsibilities

MDS NURSE (RN) Responsibilities Include:

  • Completion and submission of MDS assessments for all payer types and assessment schedules
  • Accurate coding of diagnoses, treatments, medications, functional status, and resident interviews
  • Participation in interdisciplinary care planning and IDT meetings
  • Collaboration with nursing and clinical departments regarding reimbursement and documentation needs
  • Support regulatory compliance and survey readiness efforts
  • Monitor assessment timelines and submission accuracy
  • Communicate clinical reimbursement concerns appropriately

Qualifications

MDS Nurse (RN) Qualifications:

  • RN license in the state of New York.
  • Prior MDS Coordinator / RNAC experience preferred
  • Long-Term Care and Skilled Nursing experience preferred
  • Experience with PointClickCare (PCC) preferred
  • Knowledge of RAI process, Medicare, OBRA, and reimbursement guidelines
  • Strong organizational and communication skills
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
