Position Summary:
The RN, Long Term Care provides comprehensive nursing care to residents in a long-term care setting, supporting both chronic health management and daily living needs. This role conducts ongoing assessments, monitors changes in resident condition, and coordinates individualized care plans that promote comfort, dignity, and quality of life. Working collaboratively with physicians, families, and interdisciplinary team members, the RN helps ensure timely communication, safe care transitions, and adherence to regulatory and organizational standards. As part of a supportive, team-oriented care environment, this position provides clinical leadership and oversight while fostering a stable, resident-centered setting through compassionate, consistent nursing care.
Key Responsibilities:
- Perform detailed assessments and monitor resident conditions, reporting significant changes to providers and communicating updates to families as appropriate.
- Develop, implement, and revise care plans to address resident needs, ensuring interventions support safety, comfort, and functional abilities.
- Provide and oversee direct nursing care, including medication administration, treatments, and coordination of supportive services.
- Supervise and support LPNs, CNAs, and other care staff to ensure consistent delivery of quality resident care and adherence to facility policies.
- Document assessments, interventions, and communications accurately in the electronic health record to support regulatory compliance and continuity of care.
- Participate in quality-improvement efforts, safety initiatives, and interdisciplinary meetings to enhance resident well-being and operational effectiveness.

QualificationsLicenses and Certifications Required:
- Current or temporary RN license in ND or compact RN licensure.
- BLS certification required within orientation period.
Educational Requirements:
- RN program as evidenced by licensure.
Experience Requirements:
- Previous nursing home experience preferred, but not required.
Special Skills or Training Requirements:
- Good organizational and interpersonal skills.
Physical Requirements:
The RN, Long Term Care frequently stands, walks, and assists residents with transfers, ambulation, and activities of daily living. The role requires bending, reaching, and lifting or repositioning residents with proper support and assistive devices. Adequate vision, hearing, and fine motor skills are essential for performing assessments, administering medications, and responding promptly to resident needs.
Environmental Requirements:
This role is performed in a long-term care environment with routine exposure to residents with chronic illnesses, cognitive impairments, and varying levels of mobility. Work may involve contact with bodily fluids, infection risks, and cleaning agents, requiring compliance with PPE and infection-prevention standards. The RN may encounter emotionally sensitive situations, frequent interruptions, and periods of high activity while supporting residents and staff in a team-based setting.
