This position will support the lower Eastern Cuyahoga County and Upper Summit County. Some of the citiesinclude:Independence, Maple Heights, Bedford, Solon, Novelty, Chagrin Falls, Streetsboro,Middlefieldand other surrounding cities.
As a Homecare Float RN – Weekender Flex, you will play a vital role in supporting patients by providing direct nursing care across a wide variety of needs. This position is designed to absorb evening and late-day visits, taking on uncoveredassignments,late admissions and patients added to the schedule throughout the day. Your responsibilities may include managing symptoms, drawing labs, performing wound care and wound vac changes,initiatingand educating patients on new antibiotic therapies, and ensuring seamless care transitions. In addition, you will help uphold quality standards through chart audits, supervisoryvisitsand adherence to agency protocols—making a meaningful impact for patients and their families when they need it most.
This is aweekenderflex position.A caregiver in this position works every weekend and 1 weekday per week from 10:00AM – 10:30PM.
A caregiver who excels in this role will:
Provide direct nursing carein accordance withestablished policies,proceduresand protocols.
Monitor quality through a variety of means such as chart audits and supervisory visits.
Participate in and support quality improvement projects as well asparticipateindepartmentand committees as assigned.
Supervise starts of care, resumptions ofcareand routine visits asrequested
Performaccurateand complete admission assessments and reassessments within specifiedtime frames
Assess patients for safety risks,abuseand neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess and document patients’ pain using pain scales and managing patients’ pain.
Maintain complete and up-to-satedocumentation and files at alltimeregardingpatient condition and effectiveness of care.
Escalate change in patient condition to patient’s physician asappropriate
Minimum qualifications for the ideal future caregiver include:
Graduate of an accredited school of nursing (Diploma,ADNor BSN program)
Current licensure as a RN in the State of Ohio
Basic Life Support (BLS) certification through the American Heart Association (AHA)or American Red Cross
Valid Ohio Driver’s License upon hire; per policy 8-175 thereafter
Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter
One yearofcurrent clinical experience as a RN
One yearofexperience in an acute medical-surgical or home care setting
Preferred qualifications for the ideal future caregiver include:
Bachelor’sof Science in Nursing (BSN)

Physical Requirements:
Requires full range of motion, manual and finger dexterity and eye-hand coordination.
Requires standing and walking for extensive periods of time.
Requires corrected hearing and vision to normal range.
Requires some exposure to communicable disease or bodily fluids.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
Must be able to read, write and communicate with patients.
Personal Protective Equipment:
Follows standard precautions using personal protective equipment as required.
