The Home Infusion Services-RN provides specialized nursing care and case management for patients receiving infusion therapy in the home setting. Under the supervision of the Nursing Manager, this role coordinates care, performs clinical assessments and treatments, monitors patient outcomes, and educates patients and caregivers to support safe, high-quality care.

Graduate of an accredited school of Nursing
Minimum of two (2) years of experience as a Registered Nurse
Demonstrated competency in peripheral IV insertion and management of vascular access devices, including PICCs, implanted ports, and central venous catheters
Experience performing patient assessments, medication administration, and clinical documentation
Ability to travel to patient homes and other designated care locations within the service area
Current Registered Nurse (RN) licensure in Texas or eligibility to practice in Texas through the Nurse Licensure Compact (NLC). and
(BLS) BASIC LIFE SUPPORT certification from the American Heart Association (AHA) or American Red Cross (ARC) and
Valid Texas driver's license and current automobile insurance and
Successful completion of the Oncology Nursing Society/Oncology Nursing Certification Corporation (ONS/ONCC) Chemotherapy and Immunotherapy Certificate Course within 60 days of hire
Bachelor of Science in Nursing (BSN)
One (1) or more years of home infusion, ambulatory infusion, oncology infusion, home health, or specialty infusion nursing experience
Experience administering specialty infusion therapies, including biologics, immunoglobulins (IVIG), anti-infectives, parenteral nutrition (TPN), chemotherapy, immunotherapy, and pain management therapies
Experience providing patient and caregiver education related to infusion therapy and vascular access device care
Experience collaborating with interdisciplinary healthcare teams
Experience using electronic health records and documentation systems in home health, outpatient, or infusion settings
JOB DUTIESPerforms comprehensive nursing assessments and delivers infusion therapy in the home setting, including management of vascular access devices, monitoring for adverse reactions, evaluating treatment effectiveness, coordinating care across the interdisciplinary team, and educating patients and caregivers to promote safe, effective, and patient-centered care.Conducts comprehensive patient assessments before, during, and after infusion therapy.Reviews medical history, allergies, laboratory results, and current medications to ensure safe administration of prescribed therapies.Establishes and maintains peripheral IV access and demonstrates proficiency in the assessment, access, care, and troubleshooting of vascular access devices, including PICCs, implanted ports, and central venous catheters.Monitors for infusion-related reactions and adverse events and initiates appropriate interventions.Evaluates patient response to treatment and communicates findings to providers and pharmacists.Develops, coordinates, and updates individualized plans of care in collaboration with providers, pharmacists, patients, and caregivers.Coordinates services across the interdisciplinary team to ensure timely initiation and continuation of therapy.Facilitates transitions of care and continuity of services across inpatient, outpatient, and home settings.Identifies barriers to treatment adherence and implements strategies to optimize outcomes.Verifies prescribed medications, dosage, administration schedules, and infusion parameters prior to therapy.Ensures safe handling, storage, transportation, and administration of infusion medications according to organizational standards and regulatory requirements.Adheres to infection prevention and aseptic techniques during medication administration and vascular access management.Conducts follow-up assessments through home visits, telephone outreach, telehealth encounters, or other approved communication methods.Monitors clinical progress, laboratory results, and therapy outcomes and notifies providers of significant findings.Assesses treatment adherence and patient understanding of the prescribed therapy plan.Provides comprehensive patient education regarding therapy goals, medications, infusion administration, vascular access care, potential adverse effects, infection prevention, and emergency response procedures.Provides patient/family education procedural instructions, general health information, and reinforces physician instructions in person and through telephone contact and documents patients‿ response in medical record.Assesses patient and caregiver competency and readiness to independently manage therapy in the home setting.Maintains accurate and timely clinical documentation, including assessments, interventions, patient responses, adverse events, and care coordination activities.Completes documentation in accordance with organizational policies, payer requirements, accreditation standards, and state and federal regulations.Collaborates with physicians, advanced practice providers, pharmacists, dietitians, social workers, and other healthcare professionals to optimize patient outcomes.Participates in multidisciplinary case reviews and care planning conferences.Participates in quality improvement initiatives, performance improvement activities, and patient safety programs.Reports and investigates adverse events, medication errors, near misses, and safety concerns in accordance with organizational policy.Assesses the home environment for safety, infection prevention, equipment functionality, and appropriateness for home infusion therapy.Identifies and addresses environmental factors that may affect treatment success or patient safety.Utilizes approved telehealth, remote monitoring, and electronic health technologies to support patient care, care coordination, and clinical decision-making.Travels to patient care locations as assigned and provides vascular access services across the health system and community service area.Reviews and responds to remote patient monitoring data when applicable.Participates in on-call rotation as assigned.Performs other duties as assigned.Salary Information
