
CPIhealth is currently seeking to fill an experienced Home Infusion RN that will complement and enhance the quality of service to our patients, providing information and direction in a professional, courteous and timely manner. The Home Infusion Nurse is involved in strategic planning and continuous quality improvement by providing planning and development of patient care. Ensures and maintains patient care delivery in compliance with all local, state and federal regulations and department objectives. Assure adequate exchange of information and communication among teams. This role is intended to serve patients in their home. No evenings or weekends and no on-call! Critical care experience preferred.
Essential Functions:
1. Medical treatment coordination for prospective and/or current patients with implantable pumps.
2. Performs pump refills under direction of Advanced Practice Provider (APP) or physician in the patient home.
3. Prepare home care orders and any other documentation for approval signatures; mail, fax, or electronically document to appropriate destinations and / or providers.
4. Triage implant calls as needed.
5. Daily coordination of treatment and/or triage issues with office implant nursing, provider, and support staff.
6. Communicate with specific departments via tasking in EHR (NextGen and CareTend).
7. Collaborates with APPs and Physicians regarding pump medication prescriptions.
8. Research and complete any device documentation report forms.
9. Perform and document patient assessment.
10. Education and collaboration of patients and family members from consult to post procedure.
11. Assists implant physi
cians in completing patient forms, and or insurance/prior authorization for procedures or medications when applicable and document.
12. Helps develop and update implant templates and databases.
13. Accurate and complete EHR documentation.
14. Demonstrates knowledge of nursing process- assessment, intervention, and education (including pain assessment, reassessment, management, and patient / family education regarding pain).
15. Develops, revises, and updates patient’s care plan as needed.
16. Reports changes to the APP, physician, and or pharmacist.
17. Monitors lab reports. Reports lab results to APP, physician, and or pharmacist as needed.
18. Demonstrates a thorough understanding of infection prevention and control processes. Follows standard precautions and aseptic technique at all times.
19. Participates in required meetings, trainings, and committees.
20. Provides patients with courteous, friendly, and efficient service.
21. Maintains patient confidentiality.
22. Assists with training of other Home Infusion Nursing staff related to job duties.
23. Reads, follows, and applies all appropriate procedures and protocols.
24. Performs other duties as assigned to support the mission, values, and strategies of CPIhealth.
Requirements
Knowledge, Experience, and Qualifications:
1. Successful completion of an accredited Registered Nursing program.
2. At least 10 years of related experience preferred.
3. At least 1 year of previous home care experience.
4. Strong organizational, judgement, communication, and analytical skills.
5. Ability to multi-task and perform multiple priorities.
6. Cooperative and professional behavior demonstrated with patients, peers, providers, leadership team, and others.
7. Ability to independently make decisions and solve problems; ability to apply general rules to specific problems to produce answers that make sense.
8. Evidence of leadership qualities. Consistently contributes to the improvement of Nura Home Infusion and committee participation.
9. The ability to contribute in a team environment while providing excellent customer care.
10. Highly proficient with MS Office (Word, Excel, Access, and PowerPoint).
11. Working knowledge of NextGen and CareTend.
Licensure and Certifications:
1. Current Kentucky RN license.
2. BLS certified or obtains certification within three (3) months of hire.
3. Valid Driver's License.
4. Current and adequate automobile insurance policy.
