Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes.

As part of the East 2 team in this role, you will facilitate high quality nursing care to patients in the comfort of their own homes throughout the entirety of eastern Cuyahoga County as well as Lake and Geauga Counties. You will provide assessments, complex nursing treatments and education to help them avoid readmissions. This position offers a unique opportunity to meet patients in a familiar setting where they feel safe with being open, vulnerable and receptive.

A caregiver in this position works 10:00am to 10:30pm on 3 12-hr shifts. No weekends, holidays or on-call requirements.

A caregiver who excels in this role will:

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the agency, taking visits on the uncovered list, late admissions and other patient visits as needed.
  • Participate in and support quality improvement projects.
  • Participate in departments and committees as assigned.
  • Supervise starts of care, resumptions of care and routine visits as requested.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patient for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.

Minimum qualifications for the ideal future caregiver include:

  • Graduate from an accredited School of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) through the American Heart Association (AHA) or American Red Cross
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical/surgical or homecare setting
  • Valid Driver’s License (VDL) and proof of 100,000/300,000 automobile insurance coverage

Preferred qualifications for the ideal future caregiver include:

  • Bachelor’s of Science in Nursing (BSN)

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires corrected hearing and vision to normal range.
  • Requires some exposure to communicable disease or bodily fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.
  • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Must be able to read, write and communicate with patients.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
