Job Title: RN Visit Nurse | Western Twin Cities Home Health | $4,000 Sign-On Bonus Company: Home Health Care, Inc. At Home Health Care, Inc. , we're looking for an RN who wants to take their clinical skills into homes throughout the western metro and help patients recover, manage their health, and maintain their independence. One Patient at a Time Changes the Way You Nurse. Walk into the home. Sit down with your patient. Assess what's actually happening. Look at the medications they're really taking. See how they're managing their condition between appointments. Talk with the family member helping provide care. Notice the things that might never show up during a short office appointment. Then use your nursing judgment to determine what happens next. That's the advantage of providing care where your patients actually live. Your visits can help identify problems earlier, improve patient understanding, prevent complications, and keep someone safely at home instead of returning to the hospital. A Territory Built Around the Western Metro You'll provide skilled nursing visits throughout communities in the western Twin Cities and Lake Minnetonka area , including areas such as: Minnetonka, Wayzata, Mound, Orono, Delano, Watertown, Victoria, Carver, and surrounding western communities. Rather than working inside the same building every day, your work takes you directly into the communities where your patients need you. What You'll Be Doing Provide skilled home health nursing visits in patients' homes Complete comprehensive nursing assessments and ongoing evaluations Perform wound care, medication management, injections , and other skilled nursing services Monitor acute and chronic medical conditions Identify changes in condition and intervene appropriately Educate patients and caregivers about medications, diagnoses, treatments, and disease management Coordinate care with physicians and other healthcare providers Collaborate with PT, OT, SLP, social work, and interdisciplinary team members Help patients safely transition home following hospitalization or rehabilitation Complete timely documentation using Homecare Homebase (HCHB) Help reduce unnecessary emergency department visits and rehospitalizations You Have Independence. You Don't Practice Alone. Home health requires confidence. You're the clinician walking through the patient's door and assessing what's happening in real time. But independence shouldn't mean being left on an island. You'll have an experienced clinical team behind you when questions arise, patient conditions change, or additional resources are needed. If you're an experienced RN who is clinically confident, organized, comfortable working independently, and ready for something beyond traditional bedside nursing, we'd like to talk.
