The Cystic Fibrosis (CF) Clinic is part of the Pulmonary Division at Oregon Health & Science University (OHSU) and provides highly specialized, comprehensive care for individuals living with cystic fibrosis. The clinic supports patients across the continuum of CF care, addressing the complex and evolving medical, treatment, and supportive needs associated with this chronic multisystem disease.
The CF care team is multidisciplinary and includes physicians, advanced practice providers, nurses, respiratory therapists, dietitians, social workers, pharmacists, and other healthcare professionals. The team collaborates to provide evidence-based, coordinated, and patient-centered care focused on optimizing health outcomes, supporting treatment adherence and self-management, and improving quality of life for individuals and families living with CF.
The Cystic Fibrosis (CF) Registered Nurse (RN) is an integral member of the multidisciplinary CF care team, providing specialized nursing care, patient and family education, care coordination, and clinical support to individuals living with cystic fibrosis across the continuum of care. The RN works closely with CF providers and members of the interdisciplinary team to promote safe, timely, evidence-based care and support patients and families in managing the complex medical, psychosocial, and treatment needs associated with CF.
Using professional nursing judgment and the nursing process, the RN provides comprehensive assessment, clinical management, patient and family education, triage, and care coordination for individuals living with CF. The RN addresses acute and ongoing clinical needs, supports CF self-management and treatment adherence, and coordinates care, testing, referrals, medications, and other services based on individual patient needs. This includes supporting scheduled CF clinic visits, annual CF evaluations, follow-up care, and ongoing communication with patients and families through telephone, electronic messaging, and other appropriate modalities.
The RN supports continuity of CF care across inpatient and ambulatory settings, including hospital admissions and transitions of care. The RN collaborates with inpatient nursing, pharmacies, insurance providers, social work, nutrition, respiratory therapy, and other healthcare professionals to address barriers to treatment and support timely access to medications, therapies, equipment, and other resources. The RN also contributes to CF program quality improvement, evidence-based practice, clinical workflows, patient education resources, and program standards, and serves as a clinical resource to colleagues. The RN demonstrates professional nursing leadership through clinical expertise, collaboration, mentorship, precepting, committee or shared governance participation, and contributions to initiatives that improve patient safety, clinical outcomes, continuity of care, and the patient and family experience.
