Salary: $115,000 – $150,000 (DOE)

Health Link Home Health and Hospice has been providing high-quality home health services throughout the San Francisco Bay Area and East Bay for nearly 20 years. We’re seeking a self-driven, compassionate RN Clinical Supervisor to join our dynamic team in Walnut Creek.

Supervise and support Clinical Coordinators to ensure optimal patient care
Oversee individualized patient care plans and ensure timely updates
Address staff and patient inquiries, offering clinical guidance
Triage patient calls and prioritize clinical concerns
Implement and enforce home health clinical policies and procedures
Ensure accurate, timely documentation and compliance with regulations
Active California RN License (Required)
Home Health experience (Required)
1–2 years of supervisory experience (Preferred)
BLS Certification (Preferred)
Excellent organizational, communication, and interpersonal skills
Strong computer proficiency (Apple products & EMR systems)
Ability to multitask and manage high call volumes with professionalism
Associate Degree in Nursing (Required; BSN Preferred)
One on-site location in Walnut Creek, CA
No remote work
