RN - Clinical Care Manager

Classification: Exempt

Reports to: Health Services Director

Touchmark on West Prospect is Appleton’s leading full service senior living community, dedicated to enriching the lives of residents through exceptional care, hospitality, and meaningful relationships.

We are seeking an experienced Registered Nurse to serve as our RN Clinical Care Manager, providing clinical leadership and oversight for our Assisted Living and Memory Care neighborhoods. This role combines the hands-on clinical expertise of an RN with the leadership responsibilities of a Clinical Care Manager, with a strong emphasis on resident assessment, clinical oversight, quality of care, and coordination of services.

What You’ll Do

  • Provide clinical leadership and oversight to ensure residents receive high-quality, person-centered care
  • Assess residents and monitor changes in health status, including completing appropriate documentation and follow-up
  • Develop, review, and update resident service plans based on current clinical needs
  • Coordinate resident care with physicians, providers, families, pharmacies, and other health care partners
  • Oversee medication management and ensure physician orders and service plans are followed
  • Provide medications, treatments, and direct nursing care when clinically necessary
  • Support preassessments, new resident move ins, and transitions between levels of care
  • Monitor clinical documentation for accuracy, completeness, and regulatory compliance
  • Provide clinical guidance, education, delegation, and training to Health Services team members
  • Observe care delivery and identify opportunities to improve resident outcomes and clinical practices
  • Participate in quality improvement, infection prevention, and regulatory compliance efforts
  • Serve as a trusted clinical resource for residents and families

What We’re Looking For

  • Current, unencumbered Wisconsin Registered Nurse license
  • Four or more years of nursing experience, preferably working with older adults in senior living, assisted living, memory care, or a similar health care environment
  • Previous leadership, clinical oversight, or supervisory experience preferred
  • Strong resident assessment and clinical decision-making skills
  • Knowledge of medication management and senior care practices
  • Understanding of applicable assisted living regulations and clinical compliance
  • Ability to coach, educate, and provide clinical direction to nursing and caregiving team members
  • Excellent communication skills with residents, families, providers, and team members
  • Compassion for older adults and a commitment to person centered care
  • CPR/AED and First Aid certification as required
