Are you a compassionate Registered Nurse who is passionate about providing comfort, support, and dignity to patients and families during life's most important moments? Brookestone Home Health & Hospice is seeking a Full-Time Hospice RN Case Manager to serve Holdrege and the surrounding communities

As part of the Vetter Senior Living family, Brookestone Home Health & Hospice is dedicated to delivering personalized hospice care in the comfort of our patients' homes. Our Hospice RN Case Managers play an essential role in coordinating care, managing patient needs, supporting families, and working collaboratively with an interdisciplinary team to ensure exceptional end-of-life care.

What You'll Do

• Coordinate and manage hospice care plans for assigned patients
• Perform nursing assessments and monitor patient conditions
• Provide education, guidance, and support to patients and families
• Collaborate with physicians and interdisciplinary team members
• Advocate for patient comfort, dignity, and quality of life
• Build meaningful relationships while making a lasting impact in the lives of others

Qualifications

• Current Registered Nurse (RN) license
• Current CPR certification
• Strong clinical, communication, and organizational skills
• Compassionate, patient-centered approach to care
• Positive attitude, professionalism, and personal integrity
• Home health or hospice experience preferred but not required
