Day Shift This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health is a Magnet Designated Hospital! This role will provide case management to our patients and families in the acute care setting - including med/surg, ER, ICU, and Psych Med Unit. Be a part of a collaborative and respected team. Be BOTH at Trinity Health!Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.
