Schedule:

Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | On-Call Rotation


Compensation:

$95,000 – $130,000 annually

plus
incentive visit opportunities

$6,000 in first-year bonuses

What You'll Do


  • Provide skilled nursing support across multiple home health territories throughout the Twin Cities.

  • Complete OASIS assessments and individualized plans of care.

  • Deliver one-on-one skilled nursing visits and patient education.

  • Collaborate closely with physicians and interdisciplinary team members.

  • Support local teams during staffing needs, growth, and coverage.

  • Maintain accurate and timely clinical documentation.


What We're Looking For


  • Active Minnesota
    Registered Nurse (RN)

    license.

  • Strong clinical assessment and critical thinking skills.

  • Comfortable adapting to different territories and patient populations.

  • Home health experience preferred, but not required.

  • Reliable transportation and willingness to travel throughout the metro.
