What You’ll Do
• Step into different territories to support
caseload coverage and census needs
.
• Provide
skilled nursing care, assessments, and care planning
.
• Complete
OASIS visits and coordinate interdisciplinary care
.
• Educate patients and families on
care plans and recovery goals
.
• Collaborate with physicians, therapists, and care teams.
• Travel throughout the metro while maintaining
flexibility and independence
.

What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to adapt quickly.
• Comfortable working independently in a
field-based, travel-focused role
.
• Experience in home health preferred, but we will train the right RN.
• Reliable transportation and willingness to drive throughout the metro.
