What Youll Do

As an RN Case Manager, youll be the cornerstone of compassionate care, providing support and expertise to patients in their homes, assisted living facilities, or skilled nursing homes. Your role will include:

• Serving as a trusted source of comfort and guidance for patients and their families
• Educating families on disease progression, self-care, and nutrition
• Collaborating with an interdisciplinary hospice team to develop and implement personalized care plans
• Managing symptoms with expertise and advocating fiercely for patient comfort
• Maintaining accurate, timely clinical documentation to ensure seamless care

What Were Looking For

Were seeking top-tier RNs who bring both clinical excellence and heartfelt compassion to their work. Ideal candidates have:

• A current Georgia RN license
• A valid drivers license, reliable transportation, and automobile insurance
• At least 1 year of clinical nursing experience (hospice or home health experience is a plus!)
• Strong communication skills and a commitment to patient-centered care
• The ability to manage complex cases with confidence and empathy

• Setting Outpatient hospice care in homes, assisted living, or skilled nursing facilities
